“By submitting this form, I am giving Activate my cancellation notice. I understand that my notice must be given 30 days before my renewal/invoice date. I understand that if my cancellation was given after 30 days prior to my renewal date, I will be charged for the following month. Should I choose to rejoin Activate after my cancellation is finalised, I understand that I will have to pay the current membership rates. I also understand if I have been away from the gym for longer than 8 weeks, that when I choose to re-join Activate I will be required to take some refresher sessions based on the Head Coach’s recommendations. All new members, or rejoining members will be subject to class availability and room in the gym.”
